I used Paint Shop Pro to grab a section and then put the colors for the kingdoms, and the towns and names and such on it.

It's likely to be Photoshop made, using brushes for drawing hills/mountains and a java program called "Tree-thing" for the forests.
The rest seems pure photoshop functions like offset (For shore ripples), textures, lining etc.

The thread called "Encounter at Far Point" is not an actual locaiton, it is just a Start Trek Next Generation homage to the episode of the same name, the first episode of that series
